In your case, you will need to create 5 polls and use the [ays_poll cat_id="ID"] shortcode, through which the polls will be with the next and previous buttons.

I want to mention, that the plugin itself doesn’t have any limitations.
You can add as many image answers as you want. The case mostly depends on your Server parameters and limits.
